For emergency shelter

Call 3-1-1. This is not an immediate service. Notify the operator if you have special needs.

For the Street Medicine Team serving unhoused individuals

Call the Street Medicine Outreach Team at

773-256-7549.

For shelter placements and interim basic services

Visit the Shelter Placement and Resource Center (SPARC) at 2241 S. Halsted Street. SPARC is a 24-hour intake shelter for adults. Drop in or call 3-1-1 to check bed availability.

For the Health Outreach Bus Team serving unhoused and unstably housed individuals

Where we help all ages:

Street Medicine Team: travels to sites across the city, Monday through Saturday. Call 773-256-7549

Harold Washington Library: drop-in hours offered on Mondays for access to case workers and health/mental health care.

Health Outreach Bus: brings medical care and community to underserved areas of Chicago, Sunday through Thursday.

CTA Outreach: connects with unhoused individuals at public transit locations, providing essential resources.

What We Offer all ages:

Case Workers: Help with housing, shelters, rehab centers, navigating government programs, and practical advice.

Harm Reduction: Harm reduction kits with safe-use supplies, substance use treatment referrals, and behavioral health support available.

Health Care: Basic medical care; Blood pressure & blood sugar checks; Referrals for medical and dental care; Wound care, medications, and chronic conditions.

Human Connection: Conversation, connections, food, water, and basic supplies.
