A. Anne Holcomb, Continuing Care Coordinator, provides proactive continuing care to previous residents of Open Door Shelter - West Town and helps them make a successful transition to their next living situation. She sets up transition planning meetings with each resident and his/her Case Manager prior to discharge. She makes follow-up visits to assess client needs; distribute supplies and make referrals as needed.

Additionally, she works with our Youth Outreach Team at youth outreach events, and co-leads H.E.L. L. O. (Homeless Experts Living Life's Obstacles), a youth activism group which meets Tuesday nights at the Broadway Youth Center. The activism group focuses on helping homeless and formerly homeless youth find their voices and speak out on their own behalf.
