Our Continuing Care Program provides services including:

- Home visits when the youth enters an independent living program or an apartment
- Referrals so that youth continue making progress after they leave the shelter in their new environments
- Life skills education (which starts when youth reside at Open Door Youth Shelter - West Town and continues after they leave)
- Safe sex and pregnancy prevention education
- Education on active citizenship and how to advocate for oneself
- Positive youth development opportunities: field trips, art shows, advocacy opportunities
- Assistance with:

• Food
• Transportation
• Basic hygiene items
• Personal identification
• Household set-up
