Continuing Care Coordinator is here for youth who move on from the Interim Housing Program and Transitional Living Program located at Open Door Youth
Shelter - West Town.
Our former residents face the accumulated effects of complex, multiple life
challenges. Through Continuing Care, we maintain supportive relationships with
former residents and provide access to resources as they transition to self-sufficient
young adulthood.
Our Continuing Care Coordinator begins building relationships with each youth
in our 120-day Interim Program and Transitional Living Program.
Before leaving, the youth meets with the Continuing Care Coordinator
to make follow-up arrangements. When youth leave Open Door, their relationship
with The Night Ministry is transferred from their Case Manager to the Continuing
Care Coordinator.
