Case Managers from the 120-day Interim Housing Program and Response-Ability Pregnant & Parenting Program identify clients who might be candidates for the Transitional Living Program. Candidates meet and exceed their program goals. Clients under age 18 must have parental or guardian permission to reside in this program.
Case managers assist their client with the Transitional Living Program application, which the Case Manager reviews.
If there is room in the program and the Transitional Living Program Case Manager believes it’s a strong application, then an interview is set up with the Transitional Living Program Manager and the Transitional Living Program Case Manager. The Open Door Youth Shelter – West Town Director or Assistant Director also provide input in the selection process.
After passing the interview process, candidates are granted an overnight stay to allow for the Independent Living Coaches to assess their ability to adapt to the new environment, rules and expectations.
After the overnight, staff meet to discuss case transition possibilities. If a positive decision is reached, a move-in date is set for the client.
